Game plans

I keep hearing commentators and players talking about game plans. Players saying we stick to our game plans, and commentators and analysts saying, some teams have no game plan.

What are examples of game plans which can be implemented for a team to be successful ?
And which game plans usually don't work ?

I think essentially you can skim it down to wether or not a team will play in a specific style using specific patterns or tactics such as NZ's Kick Pressure and Counter Attack game plan or Aussie style build the phases until you find a gap.
